A supervisor may refer an employee for a fitness-for-duty evaluation. The supervisor will make a written request to the
Employee Relations/Equal Employment Opportunity (EEO) Manager. The written request will include a copy of the employee’s job description, brief work history, and work-related problems.
All evaluations are conducted by a private contracted provider.
