This application is for organizations to propose specific services for inclusion in the Salt Lake CO-OP small business assistance program. The organization providing the service can submit this application or an existing CO-OP Business Outreach partner can use this form to request compensation for connecting businesses to a service that does not require funding (e.g., Kiva).

Note: This application is for services that are not requesting County funding. If you would like to apply to be a funded CO-OP Partner, please visit our website and follow the steps to apply.
