Report a Problem

The Salt Lake County Health Department relies on you to notify health officials of any problems you observe in the community. We can't do anything about a problem we don't know about.

To report a problem, call our public reporting line at 385-468-8888 or choose one of the options below.

The public reporting line is staffed Monday–Friday from 8:00 a.m. to 5:00 p.m.; in the case of environmental and infectious disease emergencies, callers can reach the appropriate 24-hour on-call staff person.

In most cases, public reports may be anonymous, but providing your information allows us to contact you with any questions as well as provide you with a response or resolution after our investigation.
