Request a Record

1. Complete the appropriate request form:

2. Ensure you have current, acceptable identification to prove your identity or your relation to the subject of the record.
3. Bring your completed request form, identification, and payment to one of our office locations. Vital records offices accept cash, check, money order, VISA, and MasterCard.
4. Your request is processed while you wait.

2. Copy both sides of your current, acceptable identification to prove your identity or your relation to the subject of the record (enlarge for ease of reading, if necessary).
3. Make out a check or money order for the appropriate amount to:
   SLCoHD Vital Records.
   We cannot accept credit cards for mail requests.
4. Mail your completed request form, copy of your identification, and payment to:

   Salt Lake City, UT 84111

5. Mail requests are processed and returned promptly; note that we cannot process requests without the correct payment, a fully completed request form, and a legible copy of the requester’s ID.

Use the State of Utah's online request portal, SILVER. If you live in Utah, your vital record request will be routed through your local health department.
