Community wide reporting and planning are part of the HUD CoC Grant Application requirements.  Examples of recent community-wide reports:

Salt Lake County 2018 Homeless Data Collected

• 2018 Point in Time Count Data
  • A count, required by HUD, of sheltered and unsheltered homeless persons on a single night in January
• Racial Disparity Assessment Summary
  • This summary was compiled by the HMIS team and was uploaded as an attachment to the FY2018 CoC Consolidated Application

• Tool, created by the United States Interagency Council on Homelessness, to help communities strategize and adjust their permanent supportive housing system to more efficiently end chronic homelessness.

• 2016 Housing Inventory Chart 
  • Point-in-time inventory of provider programs within a CoC that provide beds and units dedicated to serving persons who are homeless. 
• 2016 Point in Time Count Data 
  • A count, required by HUD, of sheltered and unsheltered homeless persons on a single night in January
• 2016 System Measures UT500 
  • Communities are required to measure their performance as a coordinated system. The System Measures is a report to HUD showing a community's system-level performance.
