The HealthyMe clinic is currently open by appointment only.  In an effort to keep the clinic safe during COVID-19, walk-ins are highly discouraged. Please call 385-468-0555 to schedule an appointment.

Closed for Lunch 1:00-2:00 M-Th and 12:00-1:00 on Fridays

Eligibility

The HealthyMe Employee Clinic is available to all county employees.  Employees’ family members who are enrolled in a county medical program and retirees and their family members enrolled in a county medical plan and not covered by Medicare may also use the clinic.   

Low-Cost Care

High Deductible Health Plan (HDHP)

  • Preventive Care          $0
  • Primary Care Visit     $30

Traditional PPO Plan (PPO)

  • Preventive Care          $0
  • Primary Care Visit     $10

No County Medical Plan Coverage

  • All Visits                           $30 

The above fee schedule applies to the office visit.  If you have lab work, x-rays, or other care outside the Clinic, your regular medical benefits apply and you will pay the contracted rate for those procedures.

Complete Confidentiality

Your personal health information is considered private and confidential. County has contracted the administration of the Clinic to Intermountain Medical Group, who maintains a separate and secure system for all information related to your medical record.

Intermountain Medical Group complies with all provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and subsequent amendments.

Convenient Care

The Clinic offers scheduled appointments, and often times walk-in appointments are available, so that you can be treated quickly and conveniently. Services include most everything you would see your personal physician for:

  • Well-Woman/ Man Check-ups
  • Vaccines and Immunizations
  • Strains and Sprains
  • Infections
  • Allergies

      Urgent Care

  • Cuts and Contusions
  • Sutures
  • Migraines
  • Minor Burns
  • Initial Treatment of Fractures
  • Back Pain
