The ZAP CARES Program is open to all organizations who received ZAP funding in calendar year 2020 to help offset losses due to the COVID-19 pandemic that have not already been covered by other CARES funding or other revenue sources.
Funds are intended to compensate recipients for losses incurred and expenses made because of the COVID-19 pandemic from March 1 – November 30, 2020.
Organizations will use the SmartSheet form to submit their eligibility information and required documentation. Click the purple "Submit Now" button below to open the form.
Organizations must show at least $500 in loss in revenue + COVID-related costs between March 1 - November 30, 2020 to be eligible for funding. Bank statements OR statement of activities for 2019 and 2020 must be attached to the application form along with a completed Calculated Loss Worksheet to certify loss in revenue.
The Calculated Loss Worksheet can be downloaded via the purple button below. Organizations may also include documentation for COVID-related expenses (i.e. copies of receipts or paid invoices for PPE, sanitation supplies, cleaning equipment, etc.).

Organizations will not be eligible for this funding if other sources of revenue or CARES Act relief funding meet or exceed loss in revenue and COVID-related costs (e.g. if 2020 revenue exceed 2019 revenue plus COVID-related costs). If the organization cannot demonstrate that losses plus COVID-related costs during the stated time period totaled $500 or greater, they are not eligible for this funding.
Approved organizations must be willing to enter into a written funding agreement with the County and agree to all the terms set forth in the agreement without alteration.
Salt Lake County will not disclose sensitive financial records unless required to by Utah’s Government Records Access and Management, Utah Code Ann. 63G-2-101 et seq. (“GRAMA”) or other applicable law. However, Applicant understands that all records provided to the County are subject to public disclosure under GRAMA. The County will make every effort to classify as private or protected any qualifying records.
