Get a Birth Certificate:
- If you were Born in Utah | Homeless Fee Waiver & Fact Sheet
- Born outside of Utah, but a U.S. Citizen
- If you are a Non-U.S. Citizen
Get a Social Security Card
To obtain a copy of your social security card, follow the steps below:
• You need a state issued ID card in order to obtain a Social Security Card. You can either:
• Send your ID card to the social security office
• If you do not have an ID card, visit a doctor/clinic/hospital to obtain a copy of your medical record (add link to different sites for getting a record)

• Fill out the application
• Mail in your application with your ID or copy of your medical record
• Wait 3-4 weeks
• Receive your social security card
