Before Your Appointment

Before your appointment at the travel clinic, EACH PERSON BEING SEEN must complete the packet below and bring the completed packet to their appointment. If you have appointments for four people in your group, you need to complete and bring four packets. You will hand your completed packet(s) to the registration desk when you check in for your appointment.

The packet PDF is fillable on your computer and can be printed once complete, or you can print the blank packet and fill it out in pen.

When you come to your appointment, you need to know your:

• Full itinerary (including all cities or locations you will be visiting)
• Travel plans (activities, purpose of your trip, etc.)
• Medical history
• Current medications (list in the travel packet)
• Vaccine history (access records on your smartphone via Docket)

If you are late or do not have the packet completed and the information listed above, you will need to reschedule your appointment.
