Replace Identification Card

Follow these steps to replace an expired, lost, or stolen identification card (ID).

Step 1: Complete Identification Card Application.
Important!  Print a copy of the completed application and bring it with you to your appointment.  Paper applications are available at the Department of Public Safety, Driver's License Division.

Step 2: Gather required documentation to bring with you to your appointment.
Important! You will need a Birth Certificate, Social Security Card, & two documents showing proof of Utah Residency. 

  • If your Utah residence address has changed from your current Utah ID card, provide two documents of your new Utah residence address.
  • Address changes can also be made online to avoid providing proof when coming into the office to obtain a new identification card.

Step 3: Schedule an appointment with the Department of Public Safety, Driver's License Division. 
Important! Remember to bring both appropriate documentation & copy of your completed application with you!

Step 4: Have your photo taken at your appointment.

Step 5: Pay $23 fee (non-refundable) or submit homeless fee waiver.
