If you think you became ill from a food product or restaurant, please report that to the Utah Department of Health & Human Services. Please also encourage any other ill people in your dining group to also report their illness.

Online reports are reviewed Monday through Friday, 8:00 a.m. to 5:00 p.m.; this online disease reporting form is confidential but not HIPAA-secure.
