Home & Community Based Care

Supported Aging offers several programs outlined in the Eligibility section that provide individual case management, consultations, and in-home services for:

  1. Individuals requesting assistance to continue living in an independent setting.
  2. Caregivers who are on the verge or already experiencing caregiver burnout.

We provide community based services in partnership with the State of Utah, the Veterans Administration, and other service providers, including but not limited to: Medicaid Aging Waiver, Medicaid New Choices Waiver, The Alternatives Program, and Veterans-Directed Home & Community Based Services.

  • Age requirements
  • Medicaid eligibility
  • Homebound/Frailty considerations
  • Income and/or asset limits

Basic eligibility for our programs begin with a phone intake in which you will be asked several questions regarding your ability or inability to perform independent activities of daily living such as bathing, toileting, housekeeping, meal preparation, etc.

Eligibility

Eligibility criteria for these services include age consideration, medical condition, and meeting financial requirements. For more information about service eligibility call 385-468-3280.

Medicaid eligibility is required for the Medicaid Aging and New Choices Waiver programs.

To be eligible for the Medicaid Aging Waiver you must:

  • Be 65 years of age or older
  • Meet nursing home level of care as determined by RN and Case Manager assessment
  • Meet Medicaid financial requirements

To be eligible for the Medicaid New Choices Waiver you must:

  • Be 18 years of age or older
  • Primary condition is not mental illness
  • Meet nursing home level of care as determined by RN and Case Manager assessment
  • Have been receiving Medicaid reimbursed Skilled Nursing Facility (SNF) care for 90 days or more, or reside in a licensed assisted living facility 180 days or more

To be eligible for home care through The Alternatives Program (TAP) you must:

  • Be 18 years of age or older
  • Be able to complete a phone intake to review current situation and measure the individual’s level of ability to independently perform activities of daily living to include but not limited to grocery shopping, preparing meals, getting in and out of bed, changing clothes, and medication management.
  • Income eligibility & asset limits:
    • Single individual: $1,883 maximum monthly income, no more than $6,000 in assets.
    • Married Couple: $2,555 maximum monthly income, no more than $12,000 in assets

The Veteran-Directed Home & Community Based Services (VD-HCBS) program gives Veterans of all ages the opportunity to receive the home and community based services they need in a self- directed manner. The program is available to:

  • All Veterans enrolled for VA Health Care
  • Veterans with a Primary Care Provider
  • Veterans at risk of nursing home placement (as determined by set criteria for the VA Salt Lake City Health Care System)
  • Veterans interested in directing their own care

Volunteer

Supported Aging programs provide case management and coordination of care to support homebound and medically frail clients who are either living independently in their own homes or in assisted living facilities.

Volunteer opportunities for Supported Aging are varied and based upon client needs. Volunteers for this program can expect to spend time with their assigned client doing the following:

  • Visiting
  • Helping with errands, appointments, and shopping
  • Working on crafts
  • Reading or watching a show
  • Preparing a light meal such as a sandwich or bowl of soup
  • Assistance with light housekeeping and yard maintenance if needed
  • Going to a senior center or other approved community activity

The goal of the volunteer in this placement will be to provide companionship, improved quality of life, and reduced isolation for the client.

Volunteer Requirements

Pass a background check and be at least 18 years of age and complete the volunteer onboarding process. Suggested time commitment is 2 hours per week.

Training and Support

A one on one orientation will be completed prior to placement with a client. Ongoing support will be provided by the volunteer coordinator through email, phone, and in person meetings as needed.
