Assessments & Referrals
General Info
BEFORE an individual can begin treatment he or she must first receive an assessment. Assessments for treatment can be received at any of our providers.
If you are involved in Family Dependency Drug Court, DCFS, or are HIV positive please call our office at (385) 468-4707, Monday - Friday, 8 am - 5 pm for an assessment.
If you are involved in the DORA program, call Assessment and Referral Services at (801) 587-2770 Monday - Friday, 8 am - 5 pm for an assessment.
The purpose of the assessment is to gather information on the person's functioning and strengths in all areas of his or her life. Based on the findings during the assessment, and with your (the client's) input, a recommendation for treatment is made.
Substance abuse assessments and referrals are provided by professional clinical staff. Community partners such as juvenile and adult courts, schools, hospitals, shelters, private citizens, corrections staff, attorneys, social service providers, and other County agencies refer individuals to the Division for substance abuse evaluations, as a vehicle to match clients with the appropriate level of intervention. Program staff are available for clinical consultation and inquiries from the public on issues relating to substance use and abuse.
Youth Info
For youth 17 and under, assessments for services will provide recommended level of care and can be done at any of the agencies below:
- Asian Association of Utah
- Cornerstone Counseling Center at VOA
- Odyssey House
- Salt Lake County Youth Services
- Valley Behavioral Health
