Food handler cards are awarded after attending a food handler training class approved by the Utah Department of Health and Human Services.

Replacement food handler cards are $15.

To request a replacement card, or if your card never arrived, complete the form below.

Your replacement card request will not be processed until you pay by:

Calling 385-468-3820 (Visa/Mastercard accepted); or
Mailing a check made out to "Salt Lake County Health Department" or bringing cash to:
