Permits are not transferable; when a change of ownership occurs at a permitted facility, the new owner must apply for a new permit and pay all applicable fees.

After you submit your completed application, call 385-468-3835 to pay the permit fee.
We accept Visa or Mastercard over the phone.
You may also mail a check or money order, or pay in person with cash, check, money order, Visa, or Mastercard.
The area inspector will contact you within three business days to schedule your pre-opening inspection.
Prepare for your pre-opening inspection by reviewing the requirements in the Salt Lake County Health Regulations applicable to your industry.
